Core Independent Living Services

Advocacy: Individual and Systems

CORD is proud to be a strong advocate for persons with a disability. Through education, individual assistance, working within structured systems, or outreach events and sponsored activities, CORD strives to enhance the quality of life and independence of people with a disability.

Independent Living Skills Training

CORD provides a variety of skills training activities. A comprehensive assessment is conducted and an individualized skills training program is developed for each individuals need(s).

Information and Referral

Direction, resource information and referrals and assistance to appropriate community, state and federal services are provided.

Institutional Transition to Community Living

Assistance is provided to persons who are willing and able to move into a more independent living situation in the community. CORD provides a transitional program that is designed to assist individuals residing in institutional living situations transfer to a more independent, less restrictive community-based setting.

A qualified Independent Living Services Case Worker will work one-on-one with you, exploring options to help you find the right independent living situation for a successful transition. In addition, the Case Worker will assess you healthcare requirements to ensure continuity of care is maintained.

CORD also recognizes the importance of meeting your social needs to avoid living situations that would cause isolation. Building friendships and participating in social activities help make a more positive and adaptable transition, as well as a more enjoyable life and celebration of your independence!

Peer Support

Peer support provides one-on-one counseling or group support. Peer counseling activities are an excellent avenue in keeping persons with a disability linked to other persons with a disability.
